Jessica Fink LCSW PLLC ("I," "me," or "my") respects the privacy of visitors to jessicafinktherapy.com. This policy explains what information is collected through this website, how I use and protect it, and the choices available to you.

This policy applies to information collected through the website and its consultation request form. If you become a therapy client, the use and disclosure of your protected health information will also be governed by my Notice of Privacy Practices and informed-consent documents.

Information Collected Automatically

This website is hosted by Squarespace and uses Squarespace Analytics. Squarespace may automatically collect information about visits to the website, such as:

• Pages viewed and time spent on the website

• Referring websites or search terms

• Browser, device, and operating-system information

• Approximate geographic location

• Internet Protocol (IP) address and other technical or diagnostic information

I use this information to understand how visitors find and use the website, evaluate which content is useful, maintain website security, and improve the website. I do not use Google Analytics, advertising pixels, or behavioral advertising tools on this website.

Squarespace uses cookies and similar technologies to operate the website and provide analytics. Some cookies are necessary for the website to function. Others may support analytics or additional website features.

Information You Submit Through the Consultation Form

If you request a consultation, the form may collect:

• Your name

• Email address

• Telephone number

• How you learned about the practice

• The service or services you are interested in

• Your preferred method of contact

• Your responses to eligibility and practice-policy acknowledgments

The consultation form is hosted through a Google Workspace account covered by a Business Associate Agreement with Google. Responses are stored in Google Forms and an associated Google Sheet in Google Drive. I also receive a notification through my Google Workspace email when a form is submitted. Google Forms, Google Drive, Google Sheets, and Gmail are included services under Google's applicable HIPAA Business Associate Addendum.

Submitting the consultation form does not establish a therapist-client relationship. That relationship begins only after we mutually agree to work together and the required intake and informed-consent process is completed.

How I Use Information You Submit

I use information submitted through the consultation form to:

• Respond to your inquiry

• Arrange a consultation

• Determine whether my services may be an appropriate fit

• Communicate with you about scheduling and next steps

• Maintain necessary administrative and security records

• Comply with applicable legal or professional obligations

I do not sell your personal information or use consultation-form information for targeted advertising.

Service Providers and Disclosure of Information

I use service providers to operate the website and consultation process, including:

• Squarespace, for website hosting, cookies, and website analytics

• Google Workspace, for the consultation form, response storage, and email notifications

These providers process information according to their own terms, privacy practices, and applicable agreements with my practice.

I may also disclose information:

• When you authorize or request the disclosure

• When necessary to comply with a valid legal requirement

• When necessary to protect the rights, safety, or security of you, another person, my practice, or the public

• In connection with professional, legal, compliance, or security support, subject to applicable confidentiality obligations

Retention of Consultation Inquiries

If you do not become a client, I generally retain identifiable consultation-form information for up to 12 months after the last contact and then delete or de-identify it, unless a longer period is reasonably necessary for legal, security, or administrative purposes.

I may retain de-identified business information—such as the month of an inquiry, referral source, requested service, and whether an appointment was scheduled—for longer periods. This information does not identify the person who submitted the inquiry.

If you become a client, information necessary for treatment and practice administration will be maintained in the practice's clinical record system and retained according to the applicable clinical-record retention policy. The Notice of Privacy Practices provides additional information about the handling of client records.

Data Security

I use reasonable administrative and technical safeguards intended to protect personal information. The consultation form and its responses are maintained within my BAA-covered Google Workspace environment. However, no method of internet transmission or electronic storage can be guaranteed to be completely secure.

Your Choices and Requests

You may contact me to request access to, correction of, or deletion of personal information you submitted through the website. I will respond as required by applicable law and professional obligations. Some information may need to be retained when required for legal, security, or recordkeeping purposes.

You may change your non-essential cookie preferences using the website's cookie banner or the privacy-preference control available on the website.

Adults Only

My services are intended for adults age 18 and older. I do not knowingly solicit personal information from children through this website. If you believe a child has submitted personal information, please contact me so I can review and, when appropriate, delete it.

Changes to This Policy
